SWEEPSTAKES (“SWEEPSTAKES”)
OFFICIAL RULES

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED. BY ENTERING, PARTICIPANT AGREES TO BE BOUND BY THESE OFFICIAL RULES AND THE DECISIONS OF THE SPONSOR, WHICH SHALL BE FINAL IN ALL RESPECTS.

1. ELIGIBILITY: The Sweepstakes is open only to legal U.S. residents who are (a) eighteen (18) years of age or older (19 or older if a resident of Alabama or Nevada) with a valid US driver’s license or government issued photo ID; (b) physically located in Las Vegas, Nevada at the time of entry; and (c) willing and able to accept the prize if selected as the winner. See Section 5 for prize details.

DO NOT ENTER UNLESS YOU ARE WILLING AND ABLE TO ACCEPT THE PRIZE IF YOU ARE SELECTED AS THE WINNER. THE PRIZE(S) ARE A PS5 WITH 1TB STORAGE OR OPULENT NAILS GIFT CARD.

The following persons are not eligible to participate: employees, contractors, directors, and officers of Opulent Nails Inc. (“Sponsor”), WinnerTakesAll (“Administrator”), and each of their respective affiliates, owners, subsidiaries, shareholders, officers, directors, agents, representatives and employees, both individually and collectively), each of their respective parents, affiliates, subsidiaries, distributors, participating retail locations, Web design, advertising, fulfillment, judging and promotion agencies involved in the administration, development, fulfillment and execution of this Sweepstakes (hereinafter, collectively, “Released Parties”), and each such individual’s immediate family members (mother, father, sister, brother, any child, husband, wife, grandparent, and/or “step” and their respective spouses, regardless of where they reside) and those living in their same households (those persons whether related or not who live in the same residence for at least three (3) months during the twelve-month period preceding the start date of the Sweepstakes) are not eligible to participate or win a prize.

2. SWEEPSTAKES PERIOD: Sweepstakes begins at 12:00:00 a.m. Eastern Standard Time (“EST”) on October 18,2025 and ends at 12:00:00 p.m. (noon) EST on December 19, 2025(“Sweepstakes Period”). The time clock of the computer administering this Sweepstakes is the official timekeeping device for this Sweepstakes.

3. HOW TO ENTER: There are two (2) ways to enter the Sweepstakes:

(a) PURCHASE: Participants making a purchase (of any amount) at a participating Opulent Nails Inc. location or online between the Sweepstakes Period and are the recipient of a survey invitation on their cash register receipt may complete the online survey and/or entry form to receive one (1) entry into the Sweepstakes. Upon receipt of the survey invitation, Participant must go through the website, www.onbyari.com (the “Purchase Website”) within one (1) week from the cash register receipt date or online purchase date and enter the unique code found on the cash register receipt or confirmation number through the online portal. Then, complete the survey in its entirety and click the “Submit” button. Upon clicking the “Submit” button, Participant may be prompted to complete the entry form in its entirety to include first name, last name, street address, city, state, zip, phone number and email address (if available) and click “Continue”. If prompted, entry form must be completed in order to be eligible to win. Then, complete the check boxes and click “Submit” to receive one (1) entry into the Sweepstakes. Surveys must be completed within one (1) week from the cash register receipt date or online purchase date and by 12:00:00pm EST on December 19, 2026 if the cash register receipt date is October 18, 2025 or later.

(b) WRITE-IN: To enter without making a purchase, legibly handwrite your complete name, mailing address, phone number and date of birth and mail a postcard or a card that is no smaller than 3 ½ x 5 inches and no larger than 4 ¼ x 6 inches, with proper postage to: Opulent Nails P.O BOX 184, Wappingers Falls NY 12590 All write-in entries must be postmarked by October 18, 2025 and received by February 10, 2026. There is a limit of one (1) write-in entry per envelope/postcard. Each envelope/postcard is considered one (1) entry into the Sweepstakes. Multiple entries submitted or completed by anyone other than the Participant are void. No mechanical reproductions will be accepted. Write-in entries must contain all entry fields outlined above; otherwise, they are void and will not be eligible to win. All entries must be properly received and recorded within the Sweepstakes Period in order to be eligible to win a prize.

Limit one (1) entry per person/email address per day, regardless of entry method. A day is understood to start at 12:00:00 a.m. EST and end at 11:59:59 p.m. EST on the same day with the exception of the last day of the Sweepstakes Period which shall begin at 12:00:00 a.m.EST and end at 12:00:00 p.m. EST.

All participants must have a valid email address. Multiple participants may not share the same email address. A potential winner may be required to provide identification sufficient to show that he/she is the authorized account holder of such account. Each entry must be manually key stroked and/or manually entered by the individual participant. Automated and/or repetitive electronic submission of entries (including but not limited to entries made using any script, macro, bot or sweepstakes service) will be disqualified and transmissions from these or related email addresses or IP addresses may be blocked. If it is discovered that a participant used multiple email addresses, phone numbers and/or mailing addresses or manipulated such information for the purpose of obtaining more than the allowed number of entries or prizes, that person will be disqualified and any prize won shall be forfeited.

Neither Sponsor nor any Sweepstakes Entity is responsible for any entry that is lost, late, illegible, stolen, mutilated, incomplete, invalid, unintelligible, postage-due, misdirected or undeliverable, whether due to system errors, omissions, interruption, deletions, defects, delay in operations or transmissions, theft or destruction or failures, faulty transmissions or other telecommunications malfunctions, human error, entries not received resulting from any hardware or software failures of any kind, lost or unavailable network connections, failed, incomplete or garbled computer or telephone transmissions, typographical or system errors and failures, faulty transmissions, technical malfunctions, or otherwise. Only fully completed entries are eligible. Proof of submission will not be deemed to be proof of receipt by Sponsor or Sweepstakes Entity.

4. RANDOM DRAWING: The potential winner will be selected in a random drawing on or about February 10, 2026 (after 12:00:00 p.m. EST) from among all eligible entries received by Administrator, whose decisions are final. Odds of winning depend on the number of eligible entries received. By entering the Sweepstakes, Entrants fully and unconditionally agree to be bound by these Official Rules and the decisions of the Sponsor and Sweepstakes Entities, which will be final and binding in all matters relating to the Sweepstakes.

To be eligible to receive a prize, all Entrants must have an address on file with the Sponsor or have provided an address when prompted at the time of entry. In the event that an Entrant has an incomplete address, the Entrant will be disqualified, and an alternate prize winner will be selected in a random drawing.

5. PRIZE DETAILS:

GRAND PRIZE: SIX (6) grand prize will be awarded, each consisting of a PS5 PRO 1TB. Total approximate retail value (“ARV”) of all Grand Prizes is $4,494.00.

FIRST PRIZE: Seven hundred and fifty (750) first prizes will be awarded, $100 gift card to Opulent Nails Inc. Total ARV of all First Prizes is $75,000.

Total ARV of all prizes to be awarded is $79,494.

Sweepstakes is subject to change and cancel at any time. Sponsor assumes no responsibility for canceled, delayed, suspended or rescheduled sweepstakes Winner will not receive compensation from Sponsor or Entities in the event of Sweepstakes delay, postponement, or cancellation. Prize may be subject to additional terms and conditions. Additional prize details may be provided to the winner at the time of notification.

Resale of prize is strictly prohibited. Limit of one (1) prize per person/household. All prizes are non-transferable or assigned and no substitutions or cash redemptions; except at Sponsor’s sole discretion or as otherwise provided herein. Sponsor will not replace any lost or stolen prizes. In the case of unavailability of any prize, Sponsor reserves the right to substitute a prize of equal or greater value. All Walmart gift card terms and conditions apply to the use of the prizes and are incorporated herein by reference. Each prize winner is solely responsible for all applicable federal, state and local taxes, including taxes imposed on his/her income. No more than the stated number of prizes set forth in these Official Rules will be awarded. Potential prize winners consent to the use of his/her name, likeness, biographical information, and voice in advertising worldwide without additional compensation (TN residents will not be required to sign a Publicity Release as a condition of winning a prize).

6. WINNER NOTIFICATION/CLAIMS:

GRAND PRIZE:

Potential Grand Prize winners will be notified by the Independent Administrator by email and/or phone on or about November 6, 2025 (the “Notification”). Grand Prize winners must respond to Notification within 5 business days of the Independent Administrator’s first Notification. In the event the Potential Prize Winner fails to respond to the Notification within 5 business days of the Independent Administrator’s first Notification to the potential winner or if it is determined by Sponsor and/or Independent Administrator, in their sole discretion, that a potential Prize Winner is not eligible in accordance with these Official Rules, an alternate Prize Winner will be selected at random from among the remaining non-winning eligible entries received during the Sweepstakes Period. The Potential Grand Prize winner will be required to sign and return (except where prohibited) an Affidavit of Eligibility and Liability/Publicity Release and a W-9 tax form (the “Paperwork”) within ten (10) days of the Notification. If any Grand Prize winner is considered a minor in his/her jurisdiction of residence (but must be over the age of 18), the Paperwork must be completed and signed by his/her parent or legal guardian and such Grand Prize will be delivered to minor’s parent/legal guardian and awarded in the name of parent/legal guardian. After verification of the Affidavit, Grand Prize will be mailed using the United States Postal Service Priority Mail. Potential Grand Prize winner must return the Paperwork within the date and time indicated in the Notification. If a potential Grand Prize winner fails to return the Paperwork within the date and time indicated in the Notification, or is found to be ineligible, or if he/she does not comply with the Official Rules, such potential Grand Prize winner and his/her entry will be disqualified and another potential Grand Prize winner will be selected from the remaining eligible entries received. If the Notification is returned as undeliverable, this will result in disqualification, and an alternate Grand Prize winner will be selected from all remaining eligible entries.

The value associated with the Grand Prize is taxable as income and the Grand Prize winner is solely responsible for any taxes, including, but not limited to all applicable federal, state and local taxes that become due with respect to the value of the Grand Prize. The Sponsor must report the value of the Grand Prize to the Internal Revenue Service in the year the Grand Prize is received by the Grand Prize winner and will be reported to the Grand Prize winner and the Internal Revenue Service in the form of a Form 1099.

Grand Prize winners will be shipped their Grand Prize within 4 weeks from receipt of the signed Paperwork.

FIRST PRIZE:

Potential First Prize winners winning a First Prize will be shipped their First Prize within 4 to 6 weeks from the drawing date via the United States Postal Service to the address on file with the Sponsor to the address supplied at the time of entry. In the event the First Prize is returned, the address is insufficient or incomplete for shipping purposes, or undeliverable for any reason, the First Prize winner will be disqualified, and an alternate First Prize winner will be selected from the remaining eligible entries received. If the alternate First Prize winner’s First Prize is returned, the address is insufficient or incomplete for shipping purposes, or undeliverable for any reason, the alternate First Prize winner will be disqualified, and the First Prize will not be awarded.

No compensation will be provided for any prizes that are not able to be awarded to a prize winner.

7. LIMITATIONS OF LIABILITY AND RELEASE: By participating, Entrants and prize winners agree to release, discharge, indemnify and hold harmless the Sponsor, the Sweepstakes Entities, and each of their respective officers, directors, employees, representatives and agents (collectively, the “Released Parties”) from and against any claims made by any Entrant, prize winner, or any other third parties, related in any way to the operation of this Sweepstakes as well as any other claims, damages or liability due to any injuries, damages or losses to any person (including death) or property of any kind resulting in whole or in part, directly or indirectly, from receipt, acceptance, possession, misuse or use of a prize or participation in any promotion related activity or participation in this Sweepstakes.

No liability or responsibility is assumed by the Released Parties resulting from participant's participation in or attempt to participate in the Sweepstakes or ability or inability to upload or download any information in connection with the Sweepstakes. No responsibility or liability is assumed by the Released Parties for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the Sweepstakes: Hardware or software errors; faulty computer, telephone, cable, satellite, network, electronic, wireless or Internet connectivity or other online or network communication problems; errors or limitations of any Internet or wireless service providers, servers, hosts or other providers; garbled, jumbled or faulty data transmissions; failure of any online transmissions to be sent or received; lost, late, delayed or intercepted transmissions; inaccessibility of the Website in whole or in part for any reason; traffic congestion on the Internet or Website; unauthorized human or non-human intervention of the operation of the Sweepstakes, including without limitation, unauthorized tampering, hacking, theft, virus, bugs, or worms; or destruction of any aspect of the Sweepstakes, or loss, miscount, misdirection, inaccessibility or unavailability of the Website or a participant’s email account used in connection with the Sweepstakes. The Released Parties are not responsible for any printing, typographical, technical, computer, network or human error which may occur in the administration of the Sweepstakes, the uploading, the processing of entries, the announcement of the prizes or in any Sweepstakes-related materials. Use of Website is at user’s own risk. The Released Parties are not responsible for any personal injury or property damage or losses of any kind which may be sustained to user's or any other person's computer or web-enabled device resulting from participation in the Sweepstakes. In the event of an identity dispute of a participant, the entry will be considered made by the authorized account holder of the email account the entry is submitted from at the time of entry. The “authorized account holder” is the person who is assigned to the email account by the online service provider. JURISDICTIONS DO NOT ALLOW THE LIMITATION OR EXCLUSION OF LIABILITY FOR INCIDENTAL, CONSEQUENTIAL, OR OTHER DAMAGES; AS A RESULT, THE ABOVE LIMITATIONS OR EXCLUSIONS MAY NOT APPLY TO YOU, AND THE FOREGOING PARAGRAPHS SHALL NOT APPLY TO A RESIDENT OF NEW JERSEY TO THE EXTENT DAMAGES TO SUCH NEW JERSEY RESIDENT ARE THE RESULT OF THE RELEASED PARTIES’ NEGLIGENT, FRAUDULENT OR RECKLESS ACT(S) OR INTENTIONAL MISCONDUCT.

8. RELEASE OF LIABILITY: By participating in the Sweepstakes, participants agree (a) to release the Released Parties, from any and all claims or actions of any kind whatsoever for injuries, damages or losses to persons and property, including bodily injury and death, arising from or relating to such participant's participation or inability to participate in the Sweepstakes; (b) under no circumstances will participant be permitted to obtain awards for, and participant hereby waives all rights to claim, punitive, incidental, consequential, or any other damages, other than for actual out-of-pocket expenses; (c) all causes of action arising out of or connected with this Sweepstakes, or any prize awarded, shall be resolved individually, without resort to any form of class action; and (d) any and all claims, judgments, and award shall be limited to actual out-of-pocket costs incurred, excluding attorneys’ fees and court costs. By accepting a prize, winners (a) agree that the Released Parties will have no liability whatsoever for, and shall be held harmless by winner against, any liability for injuries, losses or damages of any kind to persons or property resulting in whole or in part, directly or indirectly, from participation in the Sweepstakes; from acceptance, possession, misuse or use of any prize. The Released Parties are not liable in the event that any portion of the Sweepstakes (including any prize) is compromised, modified or canceled due to pandemics or epidemics, outbreaks of infectious disease or any other public health crisis including quarantine measures of any government authority or other restrictions, weather, fire, strike, acts of war or terrorism, government restriction, or any other condition beyond their control (a “Force Majeure Event”).

9. GENERAL CONDITIONS: This Sweepstakes is conducted in English. In the event of any discrepancy or inconsistency between the terms and conditions of these English Official Rules and the English Abbreviated Rules or other statements contained in any Sweepstakes-related materials, including, but not limited to: any entry form, survey, the Spanish version of these Official Rules and the Abbreviated Rules, and/or point of sale, television, print or online advertising; the terms and conditions of these English Official Rules shall prevail, govern and control. All federal, state and local taxes are the sole responsibility of the prize winners. By participating in this Sweepstakes, Entrants and prize winners agree to be bound by these Official Rules and the decisions of the Sponsor and Sweepstakes Entities, which are final in all respects and not subject to appeal. Sponsor reserves the right to disqualify permanently from this Sweepstakes any Entrant it believes has violated these Official Rules and verify eligibility of Entrants. Neither Sponsor nor anyone acting on its behalf will enter into any communications with any Entrant regarding this Sweepstakes, except as expressly set forth in these Official Rules. By participating in this Sweepstakes, Entrants, and prize winners release the Sponsor and each and all of the Sweepstakes Entities, from any and all liability, damages or causes of action (however named or described) with respect to or arising out of participation in the Sweepstakes, and/or the receipt or use/misuse of any prize awarded, including, without limitation, liability for personal injury, death or property damage. Failure to comply with these Official Rules may result in disqualification from the Sweepstakes. All Sweepstakes materials are subject to verification and are void if (a) not obtained in accordance with these Official Rules and through legitimate channels, (b) any part is counterfeited, altered, defective, damaged, illegible, reproduced, tampered with, mutilated or irregular in any way, (c) are obtained where prohibited, or (d) they contain printing, typographical, mechanical, or other errors. Entrants assume all risk of loss, damage, destruction, delay or misdirection of Sweepstakes materials submitted to Sponsor. In the event any portion of this Sweepstakes is compromised by activities beyond the control of the Sponsor which, in the sole opinion of the Sponsor, corrupt or impair the administration, security, fairness or proper play of the Sweepstakes or this Sweepstakes, Sponsor reserves the right at its sole discretion to modify, suspend or terminate the Sweepstakes. Winning a prize constitutes permission for Sponsor and its agencies to use prize winners' names, addresses (city & state), voices and/or likenesses for purposes of advertising, promotion and publicity, for this Sweepstakes only, without further compensation, unless prohibited by law. Sponsor may prohibit an Entrant from participating in the Sweepstakes or winning a prize if, in its sole discretion, it determines that said Entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other Entrants, Sponsor, or Sweepstakes Entities. If for any reason this Sweepstakes is not able to be conducted as planned, including, but not limited to, by reason of infection by computer virus, bugs, tampering, unauthorized intervention, fraud or any other causes beyond the reasonable control of Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes, then Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Sweepstakes and randomly draw from those entries received up to the cancellation/suspension date to award prizes

LEGAL WARNING: ANY ATTEMPT BY AN INDIVIDUAL, WHETHER OR NOT A PARTICIPANT, TO DELIBERATELY DAMAGE, DESTROY, TAMPER OR VANDALIZE A WEBSITE OR INTERFERE WITH THE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES (INCLUDING ATTORNEY’S FEES)AND DILIGENTLY PURSUE ALL REMEDIES AGAINST ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW.SPONSOR’S FAILURE TO ENFORCE ANY TERM OF THESE OFFICIAL RULES SHALL NOT CONSTITUTE A WAIVER OF THESE PROVISIONS.
